Autoinmunidad y autoinflamación en reumatología clínica, perspectivas desde el papel de las proteínas Dickkopf: una revisión temática

Antecedentes: la vía de señalización Wingless–Dickkopf interviene en numerosos procesos fisiológicos. En las últimas décadas, el interés por las proteínas Dickkopf ha aumentado especialmente en oncología, metabolismo óseo y riesgo cardiovascular. En el campo reumatológico, la evidencia se ha ampliado más allá de su papel en el daño articular destacando su posible participación en diversos desenlaces clínicos. Objetivo: sintetizar de forma descriptiva la evidencia disponible sobre la relación entre la vía de señalización Wingless-Dickkopf y las enfermedades reumatológicas, con fenómenos de autoinmunidad y autoinflamación, presentando un análisis de sus implicaciones clínicas y controversias. Metodología: se llevó a cabo una revisión temática con una búsqueda no estructurada de la literatura en bases de datos como Pubmed y Scopus. Esta se complementa con la revisión de algunas revistas especializadas en esta línea temática para la descripción del rol de las proteínas Dickkopf en un grupo de enfermedades autoinmunes y autoinflamatorias previamente seleccionadas. Resultados: las proteínas Dickkopf con mayor grado de exploración en enfermedades reumatológicas incluyen DKK1, DKK2 y DKK3. DKK1 es un mediador central del compromiso óseo; sin embargo, también se ha asociado con procesos de aterosclerosis y fibrosis tisular. De manera general, los niveles elevados de DKK1 se relacionan con desenlaces variados en patologías de origen autoinmune, aunque estas asociaciones pueden variar según el fenotipo de cada enfermedad. Por su parte, DKK2 y DKK3 parecen desempeñar roles inmunorreguladores o inmunoestimulantes dependiendo del modelo patológico estudiado. En contraste, existe información limitada acerca de la participación de DKK4 y DKKL1. Conclusiones: las proteínas Dickkopf desempeñan roles patogénicos en las enfermedades reumatológicas autoinmunes y autoinflamatorias que trascienden el daño articular. Se han identificado correlaciones clínico-biológicas relevantes para DKK1, DKK2 y DKK3, y evidencias experimentales emergentes para DKK4 y DKKL1. Su función no se limita a la inhibición de Wingless, sino que involucra mecanismos variables según la enfermedad. Se requiere que investigaciones futuras integren biomarcadores anexos de esta vía y fortalezcan la evidencia de correlación mediante modelos experimentales más diversos.

Médico, graduado de la Universidad Nacional de Colombia en 2024. Actualmente trabajo como asistente de investigación clínica en el CREA (Centro de Estudio de Enfermedades Autoinmunes) de la Universidad del Rosario en Bogotá. En este rol, participo en investigación clínica, específicamente en el seguimiento de pacientes con trastornos autoinmunes. Tengo un fuerte interés en la divulgación médica, la educación médica, el manejo de enfermedades crónicas y el estudio de lenguas extranjeras.
También completé mi servicio social en el CREA, trabajando en el proyecto "BIOMA II - Biomarcadores en Autoinmunidad", con un enfoque particular en esclerodermia. Cuento con formación básica y educación continua en el campo de la medicina interna, con intereses particulares en geriatría, endocrinología, reumatología, farmacología clínica y toxicología.
